Target Audience
Any person involved in or expected to perform accident/incident investigation or analysis in the workplace; Safety and Health Representatives, Supervisors and front line Management should attend this course.

Aim
The course is designed to develop necessary knowledge and skills for individuals to effectively perform accident and incident investigations in the workplace, manage workplace-accident and incident situations through analysis of contributing and causative factors, including making recommendations for remedial action.

Performance Objectives
Upon completion of the course, participants will be able to:
- identify and discuss the legal and ethical requirement for accident and or incident reporting and investigations in the workplace;
- demonstrate the ability to interpret accident/incident statistics and other data collected, and calculate frequency rates based on that data;
- perform an accident and or incident investigation utilising a systematic approach in evidence collection;
- determine underlying contributing (causative) factors, prepare a report; and
- express recommendations for remedial action based on the outcomes of an investigation.

Methodology
The course delivery is consistent with adult learning principals, viz:
- Multimedia presentations;
- Reference materials and workbooks as required;
- Brainstorming sessions and group dynamics;
- Discussion and consensus; and
- Facilitation of theoretical and or experiential learning.

Time-Frame
Any course is realistic in its conduct, content and approach. Interaction, discussion, variety and flexibility is stressed during skills training, however it should be noticed that this type of course could be specifically designed around an actual case and as a result the time frame will reflect the need of the client.
The course was designed, however, with an average time frame and suggested format that under normal circumstances can be taught in a total of sixteen (16) hours.
- Two (2) days based on twelve (12) participants.
- Each day would consist of at least eight (8) hours not including breaks and lunch times.

Expected Workplace Outcomes
Participants will be able to increase performance and productivity; and reduce the exposure to risk, to themselves and others in the workplace utilising the knowledge and skills learnt, specifically by:
- demonstrating their "Duty of Care" by identifying and reporting incidents and or potential hazards in the workplace;
- completing effective accident and incident investigations — based on accident and or incident reports — as required, to reduce the potential of workplace accidents/incidents through recommending effective control measures; and
- maintaining effective communication mechanisms through their elected Occupational Safety Representatives, Employers and Employees.
